Certified Allergy & Asthma Consultants would like to inform patients that Camille Zentkovich, FNP, is no longer practicing with our organization.
We appreciate Camille’s contributions during her time with our practice and thank her for the care she provided to our patients.
Please be assured that your care will continue without interruption. We are committed to ensuring a smooth transition and invite patients to continue their treatment with one of our experienced providers, including:
Patients who currently have appointments scheduled with Camille Zentkovich, FNP, will be accommodated whenever possible. If the same appointment type is available with another provider in the same office on the same date and time, the appointment will automatically be reassigned. If an appointment cannot be reassigned in this manner, our scheduling team will contact you to arrange an alternative appointment.

Our providers have full access to patients’ medical records and treatment histories, ensuring seamless continuity of care.
If you will need a school or daycare medication authorization form or note for the upcoming school year and have not had an office visit since July 1, 2025, please schedule an appointment as soon as possible. This will allow us to complete the necessary documentation and ensure your treatment plan remains up to date.

If you prefer to seek care with a provider outside of our practice, you have the right to do so. Upon your written authorization, we will promptly provide a copy of your medical records to the provider of your choice.
Please contact our office at 518-434-1446 to obtain a records release form. Completed forms may be submitted by mail, fax, or secure electronic transmission.
We encourage patients receiving ongoing treatment to establish continued care as soon as possible to avoid any disruption in treatment.
