Referrals Your Way – It’s Your Choice

Patients will receive a call within 3 business days to schedule an appointment.

• We will notify you by fax in the event we were unable to reach the patient or schedule an appt.
• Once the patient is seen, a visit note will be sent to the referring physician.

Allergies: Seasonal Allergic Rhinitis; Perennial Allergic Rhinitis; Chronic Rhinosinusitis (Allergic Sinus Disease); Bee Sting (Venom); Food Allergy; Contact Dermatitis; Penicillin Allergy

Asthma: Allergic Asthma; Non-allergic Asthma & Exercised Induced Asthma

Hives & Eczema: Urticaria/Angioedema; *Atopic Dermatitis

Immunology & Immunodeficiency: Recurrent Infections; Hypogammaglobulinemia (Low IgG); Common Variable Immunodeficiency (CVID)

*Adult Referrals require office notes and labs from PCP or Dermatologist for review prior to scheduling appointment.
