Guide to Accessing Another Patient’s Health Records via Proxy Invitation

This guide provides a step-by-step process for authorized individuals to access another patient’s health records through a proxy invitation. If you need a proxy invitation, please contact the relevant medical organization first.

Preparation for Proxy Access

Before accepting a proxy invitation, ensure you have the following:

  • A FollowMyHealth™ Universal Health Record (UHR) account. While a connection to a medical organization is not required, a personal FollowMyHealth™ login is essential.
  • If you do not have a login, you can create one during the invitation acceptance process.
  • Existing FollowMyHealth™ UHR users are already prepared to accept an invitation.

Steps for Accepting the Proxy Invitation

Upon receiving the proxy invitation, follow these steps:

Step I: Accessing the Invitation Link

Click the link in your invitation.

Step II: Creating or Logging into Your Account

If you do not have a FollowMyHealth™ UHR, select “Sign Up and Connect”. For existing users, choose “Sign in and add this connection” and log in with your FollowMyHealth™ credentials.

Step III: Completing the Sign-Up Form

For new users, fill in your demographic information on the screen that appears after selecting “Sign Up and Connect”. Remember to use your details, not the minor’s. Click “Confirm and Continue” after completing the form.

Step IV: Accepting the Terms of Use

Agree to the Terms of Use by clicking “I Accept” at the bottom of the window. Note that selecting “I Decline” will prevent you from completing the process.

Step V: Creating a Secure Username and Password

Create a FollowMyHealth™ Secure username and password as shown below:

Registering as a Proxy

Follow the subsequent steps to register as a proxy:

Step 1: Welcome Screen

You’ll be informed about the account creation. Click “Next” to continue.

Step 2: Entering the Invite Code

Enter the invite code provided in the invitation email or by the organization. Click “Next” after entering the code.

Step 3: Request for Access Agreement

Agree to the Request for Access to connect to your medical organization via FollowMyHealth™. This document is always accessible in the “Documents” section of your account.

Step 4: Uploading Health Records

Proceed with the uploading of health records.

To switch to the proxy account, click on the dropdown arrow next to “Hello”.
